			<content:encoded><![CDATA[<p>As we age the corners of the mouth start to downturn which creates a &#8220;perma-frown&#8221; and sad look.  There are different surgical procedures to correct this such as a corner-lift but these procedures create permanent visible scars that are easily visible.  My preference is to correct this problem with fillers.  Restylane and Juvederm are the two most common fillers that I use for this purpose.  Both of these are safe and easily administered in the office.  With a few injections, the filler plumps in the depression below the corner of the mouth which gives an instant lift to that area.  The results are immediate and there is no scarring.  The fillers are not permanent and do wear off between 6 and 12 months but there is often a little fibrous tissue that occurs in response to the fillers so the effect is sometimes visible for longer. 			<content:encoded><![CDATA[<p>During breast augmentation consultations patients have the opportunity to try on sizes and look at themselves in the mirror to determine the shape that they would like to see after the surgery.  I also use the Canfield Vectra3 Imaging suite to take a patients picture and then add different sized implants to their picture so that they can visualize what they can look like after surgery.  This is a nice way to &#8220;test drive&#8221; the implants before actually getting them.  I&#8217;m often asked though to achieve a particular &#8220;look&#8221; of a patient from my gallery.  A patient from the gallery may have had a 300cc implant and someone can look at the picture and comment that they look like that patient and they want the same breast shape after surgery.  The problem with this is that even though patients may have similar shaped breasts that look the same in a picture there are other differences that you can&#8217;t really tell in a picture.  These other variables have a significant impact on the final look and must be considered during the implant selection process.  Some of these variables include height, weight, band size, tissue thickness in the upper pole and lower pole of the breast, rib cage differences, breast bone position, skin elasticity, position of the breast folds, and areolar shape and position.  These factors play out when selecting an implant so sometimes a size that looks good in one patient may not give the same shape in another.  Through my consultations with patients I learn about what they are looking for and then through the physical exam and having the patient try on sizes I can put my expertise to use and select an implant that will accomplish their goal.  The &#8220;art&#8221; of breast augmentation starts outside of the operating room during the implant selection process and that is why the same breast implant can result in different looks in different patients. </p>

			<content:encoded><![CDATA[<p>Spring is an exciting time of the year.  With the awakening of the earth comes the realization that summer is just around the corner.  It does take some time to get looking your best for summer and I wanted to give an update on the recovery of some of the most common procedures.  Botox and Restylane are quick and easy office procedures that take a few minutes and have little downtime.  Maybe 1-2 days with some swelling with Restylane.  Chemical peels take 3-7 days depending on the strength.  These give a refreshed look to the skin.  Then one of my most common procedures is breast augmentation.  This surgery typically requires 2-3 days off work and about one month for exercise and unrestricted activity.  There is some swelling of the breasts initially and some fullness in the upper part of the breast for the first few weeks.  The final appearance of the breasts takes around 3 months but in clothing or a bathing suit patients look good essentially right after the procedure.  Smartlipo and Liposuction take a little longer with usually a few days off work and 1-2 weeks of feeling sore.  The shape after these procedures takes a few weeks to be seen as the swelling goes down and 3 months until results are essentially finalized.  Abdominoplasty surgery or tummy tucks require a full 2 weeks of downtime and 6 weeks or so until exercise and unrestricted activity.  This surgery makes the biggest improvement in the shape of the abdomen.  If you are thinking about getting ready to summer now is the time.  By getting your procedure in the Spring you won&#8217;t miss out.</p>

			<content:encoded><![CDATA[<p>Sometimes people ask if a tummy tuck (abdominoplasty) can be done in thin patients.  The short  answer is &#8220;Yes&#8221;.  Not everyone who would benefit from a tummy tuck has significant extra skin or fatty tissue.  One of the aspects of a tummy tuck is muscle tightening.  With pregnancy and weight fluctuations the muscles of the abdominal wall become stretched and sometimes do not snap back to their original positions.  In these cases, all the sit-ups in the world won&#8217;t allow the abdomen to become flat again.  Sit-ups will no doubt strenghten the muscles but the tummy will often still have a lower bulge.  In these cases, even if the patient is thin, they will benefit from a tummy tuck.  With a tummy tuck the muscles are tightened from the ribs down to the pubic bone.  This creates an &#8220;internal girdle&#8221; which creates a flat tummy.  The extra skin is then removed to make the tummy as smooth as possible.  With thin patients the position of the scar sometimes needs to be modified and my preference is to use a &#8220;T&#8221; at the bottom of the scar.  The reason for this is that in thin patients there may not be enough skin from the belly button to the rib cage to allow for removal of all the skin from the belly button to the pubic bone.  In this case my preference is to keep the scar as low as possible in order to keep it hidden in a two-piece bathing suit and therefor create a small vertical component at the bottom.  This will look better over the long run as opposed to a higher positioned scar on the body.  If you are thin and think that a tummy tuck is not an option for you that simply may not be the case.  The only way to find out is to come in for a consultation.</p>

			<content:encoded><![CDATA[<p>Patients often come in interested in improving the appearance of the &#8220;11&#8243; between their eyebrows.  They usually state that people ask them if they are angry when they aren&#8217;t.  The &#8220;11&#8243;s develop from muscle activity of the corrugator muscles which pull the eyebrows closer to each other.  Initially the vertical lines are present only when the muscles are moving (dynamic wrinkles) but over time the wrinkles become fixed and established (static wrinkles).  At this point the lines become visible all the time whether the muscles are moving or not.  This is important to recognize because the treatment is different if the wrinkles are fixed.</p>
<p>Botox is the baseline treatment for the &#8220;11&#8243;s but it may not be enough.  Since the muscle activity was the reason for the lines to develop in the first place it is best to first treat the muscles.  Botox weakens muscle activity so the intensity of the wrinkles will often diminish.  However, if the lines are &#8220;static&#8221; then despite having no muscle activity the lines can persist.  In this case the treatment involves adding an injectible filler such as Restylane.  This filler allows the wrinkle to literally be filled and smoothed.  The end result is a much improved and softer appearance without the angry &#8220;11&#8243;s.  With a filler the result will often last up to 6 months, however Botox alone will last for 3 months.  If you are interested in fixing your &#8220;11&#8243;s call Dr. Leedy at the Cleveland Plastic Surgery Institute.</p>

			<content:encoded><![CDATA[<p>Botox is a nice way to treat the transverse wrinkles of the forehead.  These lines develop as we age and are often the result of progressive droopiness of the brow.  As the brow drops, it puts extra weight on the eyelids which makes it harder to keep the eyes open.  In order to prevent the extra weight from the brow from pushing down on the eyelids we involuntarily lift up the brow with our forehead muscles which in turn creates the wrinkles.  The treatment for the wrinkles is Botox.  Botox works by weakening muscle activity and this prevents the forehead muscles from wrinkling the skin.  This works well as long as the forehead wrinkles aren&#8217;t too low on the forehead.  If Botox is given too low on the forehead the effect can be a smoother forehead but lower set eyebrows.  This often creates a tired or angry look and is something to avoid.  There are ways to smooth the forehead without drooping the brow but that requires an experienced, individualize approach to each patient&#8217;s anatomy.</p>
